How Do I Get a Vaccine?

Over-the-Counter COVID-19 Tests
Effective January 15, 2022, the GPC prescription drug plan will cover the cost of FDA-authorized*, over-the-counter, diagnostic COVID-19 tests for its plan members. You must be covered under a Graham medical plan to be eligible. These tests can be purchased at any in-network pharmacy without a prescription or a health assessment from a provider and are used for “At-home” testing purposes only. Members should show their BCBS IL identification card when purchasing a kit. This enhanced coverage remains effective through the end of the federal health emergency.
The benefit allows for each covered family member to receive up to eight (8) tests every 30 days. If there are two tests in a box, then the member would be allowed 4 boxes. Members who purchase the kit from an in-network pharmacy have a $0 co-pay for each test purchased (up to the limits previously described).
Members who purchase a test directly from a supplier other than an in-network pharmacy such as Walmart.com or Amazon.com, may complete a Prescription Drug Claim Form and submit for reimbursement. Members will be reimbursed up to a maximum of $12 per test. Click here to obtain the claim form.
